Preparing Health Care for Your Newborn
Congratulations on the upcoming arrival of a new member to your family! This is an exciting time that is also overwhelming. Deciding on which pediatrician best matches your needs will put you at ease.
If you are considering registering your child with our clinic, we encourage you to read our practice booklet and complete our registration forms below. Completing the forms does not commit you to using our office for the care of your newborn. However, if you do decide to choose our office for your pediatric care we can prepare a patient chart for your infant so you can learn about your baby on your first visit instead of filling out paperwork.

Feel free to call us anytime Monday - Friday 8 am to 5 pm and we can set up a prenatal consultation, schedule your first appointment with your newborn, or answer any health care related questions you may have.
